One of my first companies was a CD duplication firm, and we learned very quickly the ins and outs of digital disc care: first thing to try if a disc isn't reading, take a slightly damp microfiber cloth (or soft tissue), and wipe the disc outwards in a straight line from the center. Usually does the trick.
Other than that, I would guess a software/codec problem.. it's a little known fact but many hardware manufacturers of disc drives have inside deals with this disc producer or that disc producer; some discs will not read reliably on 'competing' companies disc drives.
